We accept private pay and the following insurances:

Insurance Company Comments/Requirements for Coverage
Blue Cross/Blue Shield HMO plans require referral from PCP.
Authorization required after 8 visits/year.
Some out-of-state plans require prior notification.
Medicare Must not be under care of VNA.
Requires physician approval.
Tufts HMO plans require physician referral from PCP.
Authorization required after 8 visits/year.
HarvardPilgrim Healthcare Requires authorization after evaluation.
Neighborhood Plan Requires authorization after evaluation.
Network Health Requires authorization after evaluation.
Unicare/Commonwealth Indemnity Plan Requires authorization after evaluation.
Requires authorization for treatment visits.
United Healthcare Somme plans require authorization after evaluation.
Aetna/US Healthcare Requires authorization after 20 visits/year.
Cigna/Connecticut General PPO, Tufts, or Carelink Plans only.
Tri-Care  
Major Carrier Networks Private Healthcare Systems, Multiplan, etc.
Workers' Compensation Requires authorization after 20 visits in any 12-month span.
All Auto Liability Plans
